In-home and telehealth care to expand on the care you get from your primary care provider and specialists

What to expect

Step 1

First, you will talk with an Equality Health team member so we can better understand your health goals.

Step 2

Then, a nurse practitioner and community health worker will visit you at home or through telehealth.

Step 3

Your nurse practitioner will set up a care plan that’s right for you. We will work with your doctors to coordinate care.

Step 4

Equality Health will provide the care and support you need to get back on track with your health.

Step 5

When you are set up for success, we’ll connect you back to your primary care provider and community resources with a solid plan in place.

Can I still see my doctor?

Yes, you can see your doctor. Our team is here to help support you and can get you back to see your PCP or your preferred specialist provider. We help support you in the short term.

How did you get my information?

We work with your health plan and your doctor.

Does my doctor know you are calling me?

Yes, your doctor is part of Equality Health Network. We can help you get back to see your doctor. The care plan we will build helps ensure they aware of all your concerns.

Can the provider refill my medications?

Yes, our Nurse Practitioner can help with your medications.

Who will be coming to my home or doing the telehealth visit?

In most cases, a Community Health Worker and Nurse Practitioner will come to your home after we set up your in-home visit. You may also receive a telehealth visit.
